Youth In Transition |
Transitions is an initiative service to assist inviduals with a developmental disability to make successful transitions into core Community Living Renfrew County South support and service programs. This is accomplished through planning based upon the individuals goals and dreams. Individuals will make informed choices about their future through the development of identified skills and exposure to a variety of community opportunities.
